one issue I'm having is with the sequence from "OpenID consumer requests authorisation" all the way to "send authorisation back"

I think that, given the provider is only intended for use in a trusted environment, usually on a LAN which is NATed from the internet, that presenting "Do you authorise this site to have your identity?" is redundant
